NECESSARY DOCUMENTS WHICH are GIVEN FOR ENTRY ON 1st COURSE
1. Application form (a form is given out in the university);
2. Original of certificate and appendix to the certificate or their the notarized copies;
3. Original of certificate Ukrainian center of education quality evaluation certificate or its notarized copy;
4. Original of medical certificate form 086- or its the notarized copy;
5. 6 photographs (size 3x4 cm);
6. A certificate copy about the appropriation of identification number;
7. Passport copy: 1, 2, 11 pages;
8. Military ticket copy (certification about the postscript to draft office) - for juveniles.
All documents originals the university applicant offers personally!
The copy of labor book, notarized by the enterprise, is additionally added by the university applicants which enter the extra-mural courses.
