  1. The application of a form;
  2. The agreement with the rector of university about improvement during 3 years in NAU after the successful ending of a preparation for doctor's degree;
  3. The personal sheet on a personnel's registration;
  4. The reassured list 12 scientific works on a subject of the future doctoral thesis published in the special collections;
  5. Statement from minutes of scientific advice NAU about:
    • The guideline in a preparation for doctor's degree;
    • The statement of a doctoral thesis subject;
    • The statement of the scientific adviser;
  6. Photocopy of the candidate of sciences diploma;
  7. The photocopy of the diploma of the senior lecturer or high scientific employee (if they are);
  8. Filled examination card of oerson working for degree of doctor;
  9. Medical certificate about a health condition under the form є 086;
  10. The work-record card (hands only after transfer in a preparation for doctor's degree);
  11. 3 photos by the size 4х6 cm;
  12. Photocopy of an identification code;
  13. The passport of Ukrainian citizen (is only presented).
