- The application of a form;
- The agreement with the rector of university about improvement during 3 years in NAU after the successful ending of a postgraduate course (for the postgraduate students of the budget form of training) or contract about paid training of the postgraduate student;
- The personal sheet on a personnel's registration;
- The reassured list is not less 2 scientific works on a subject of the future thesis published in the special collections cells or abstract on a selected scientific specialty (15 pages of the computer text with the scientific chief's marks);
- Photocopy of the diploma about a full higher education (master, specialist, engineer);
- Photocopy of the appendix to the diploma with estimations;
- The certificate hand of the chief and chief accountant with printing about a monthly average wage of the candidate in a postgraduate course of internal form training from last work place (for charge of the grant to the postgraduate students, which is by the citizens of Ukraine);
- Filled examination card of the postgraduate student;
- Medical certificate about a helth condition under the form є 086;
- 3 photos by the size 4х6 cm;
- The work-record card (for the postgraduate students of the internal form) is handed in department of a preparation for doctor's degree and postgraduate course only after transfer in a postgraduate course;
- Photocopy of an identification code (for the postgraduate students of the internal form);
- The original of the diploma about a full higher education (is only presented);
- The passport (is only presented).
